Ethics approval and consent to participate
: The study was approved by the School of Medicine Research and Ethics Committee at Makerere University, the Uganda National Council for Science and Technology, and the University of California San Francisco Committee on Human Research. Informed consent to obtain deidentified patient records were waived by the ethics review boards at both institutions. Providers, clinical staff, laboratory staff and health center leadership provided verbal informed consent which was approved by ethics review boards of both participating institutions.
