Ethics approval and consent to participate
: The National Institute of Medical Research (NIMR) - (Ref. No. NIMR/HQ/R.8a/Vol. IX/2740), granted ethical approval for this study. We obtained written informed consent in form of filling the declaration forms by the authorized persons involved in aggregation of data of this study at the regional level. Upon data submission all Regional Medical Officer, submitted a letter together with declaration forms that data were from the authentic sources, i.e. health facilities’ reports and filled checklist from regions and district councils. Finally, the Ministry of Health, Community Development, Gender, Elderly and Children and President’s Office – Regional Administration and Local Government granted permission to the data collectors to access and use the database.
