Ethics approval and consent to participate
: This research required multiple ethics approvals by various Human Research Ethics Committees (HRECs) due to the diverse organisational structures governing each of the mental health services involved in the service mapping workshops. More specifically, the governing body for Open Arms – Veterans & Families Counselling Service required applications to be submitted through the Department of Defence and Veterans’ Affairs HREC (Project numbers: E016/027 and 017–17), whilst ethics applications to conduct research with all <i>headspace</i> centres were submitted through the University of Sydney HREC (Project numbers: 2017/944 and 2018/849). All participants provided written informed consent prior to participating in this study.

: Professor Ian Hickie was an inaugural Commissioner on Australia’s National Mental Health Commission (2012–18). He is the Co-Director, Health and Policy at the Brain and Mind Centre (BMC) University of Sydney. The BMC operates an early-intervention youth services at Camperdown under contract to headspace. He is the Chief Scientific Advisor to, and a 5% equity shareholder in, InnoWell Pty Ltd. InnoWell was formed by the University of Sydney (45% equity) and PwC (Australia; 45% equity) to deliver the $30 M Australian Government-funded Project Synergy (2017–20; a three-year program for the transformation of mental health services) and to lead transformation of mental health services internationally through the use of innovative technologies. Tracey Davenport is now Director (Research and Evaluation), Design and Strategy Division, Australian Digital Health Agency. The other authors have nothing to disclose.The source of funding does not entail any potential conflict of interest for the other members of the Project Synergy Research and Development Team, or any other members of the governing bodies, such as PNHs and Lead Agencies, and health services involved in the study.
