: A waiver was obtained through the Yale Institutional Review Board since the study was classified as quality improvement research. The study was explained to all participant stakeholders, plans for dissemination, and the ability to exclude themselves at any point without repercussions. All standard measures were taken to afford privacy and information security including conducting interviews or focus groups in secluded areas, limiting duration of the interviews, and monitoring for any discomfort among participants. Stakeholders verbally consented to participate in an interview.
