All study procedures were conducted in accordance with the Tri-Council Policy Statement: Ethical Conduct for Research Involving Humans (TCPS2). Prior to conducting an interview, a description of the study was provided, and written or verbal informed consent was obtained from all participants. Since some participants had low literacy levels, the option of verbal informed consent was provided. The procedure for obtaining verbal informed consent followed the protocol approved by the Research Ethics Board at the University of Waterloo (reference number listed above).
