: All methods were carried out in accordance with relevant guidelines and regulations. The data collection for the first phase was approved by the Regional Ethical Review Board in Linköping, Sweden (Ref no: 2017/181 − 31 and 2017/564 − 32), and the data collection for the second phase was approved by the Regional Ethical Review Board in Umeå, Sweden (Ref no: 2020–02548 and 2020–04862). A returned questionnaire was considered as informed consent in phase I. In phase II participants gave their informed consent by answering a mandatory question in the web survey.
