: Our survey study involving human participants was in accordance with the ethical standards of the institutional research committee and with the 1964 Helsinki Declaration and its later amendments or comparable ethical standards. The study was approved by University of Pittsburgh Institutional Review Board (IRB). The University of Pittsburgh IRB provided a waiver to document informed consent under the criteria that “the research presents no more than minimal risk of harm to the subjects and involves no procedures for which written consent is normally required outside of a research context”. Research assistants verbally reviewed a standard script and obtained verbal consent from each participant and documented this electronically. The first page of the self-administered electronic survey also presented the elements of the standard script for participants to review in written form before proceeding with the survey. d the informed consent form written on the front to read, by pressing next on the survey, consent was documented.
