: Participation of children in the pragmatic clinical trial described here was based on written informed parental consent. Study was evaluated and all experimental protocols were approved by Regional committees for medical and health research ethics in Norway (reference 2016/2002/REK). Participation in the questionnaire was voluntary and consent based (written consent), and no compensation was given to the respondents. Anonymity of the respondents was ensured by QuestBack. All methods were carried out in accordance with relevant guidelines and regulations. The study was evaluated and all experimental protocols were approved by data protection officer at Viken County and the Norwegian Centre for Research Data (reference 727424).
