: The study (IRB approval number: IRB2015-08-0561) was ethically approved by Institutional Review Board of Fudan University School of Public Health, which was registered with the Office for Human Research Protections, IRB00002408, and has a Federal wide Assurance, FWA00002399. Informed consent was collected verbally for all interviewees, since this study is an interview-based survey study in a semi-structured conversation about the day-to-day management practices and no interventions were implemented on study interviewees. All survey methods regarding with informed consent were pre-specified in the study protocol and were carried out in accordance with the survey guidelines originally designed by the Institute of Economic Policy Research, Stanford University, and regulations of the ethical review committee.
