: The Health Research Ethics Committee at Stellenbosch University gave ethical approval for the study (N20/09/092), which was conducted according to the principles of the Declaration of Helsinki. The Western Cape Department of Health gave permission for the study to be conducted.Patients were invited by community health workers and if they agreed to participate their phone number was given to the researchers. When they were phoned they gave verbal informed consent prior to the questions. Consent was confirmed in the first question of the questionnaire and captured in REDCap. Written informed consent was not possible because we could not perform face to face interviews during the COVID-19 pandemic and patients did not have email. All other participants gave written informed consent. This was approved during independent review by the Health Research Ethics Committee at Stellenbosch University.
