: The ethical approval for the study has been obtained from the Research Ethics Committee of THL (excerpt of proceedings 5/2018 (§791)). All methods were carried out in accordance with the Declaration of Helsinki. According to Finnish legislation, no informed consent is required for studies using secondary register data only. Permissions to use data were applied from the register maintainers including the Finnish Institute for Health and Welfare (THL), the Social Insurance Institution of Finland (KELA), Statistics Finland, and the Kuopio University Hospital. Researchers signed confidentiality obligations and had access only to the pseudonymized data without direct identifiers.
