: This study has been ethically reviewed and approved by the Muhimbili University of Health and Allied Sciences and the National Health Research Ethics committee in Tanzania as well as the Regional Ethical Committee (REK) in Norway. Permission to conduct the study was sought from local government authorities. Information about the study was provided to participants and written consents were obtained from all participants before enrolment into the study. All research activities were carried out in accordance with guidelines and regulations of Tanzania National Health Research Ethics Committee (NatHREC) and Regional Committee for Medical and Health Research (REK) in Norway.
