: This study protocols, documents, and forms used in it were approved by the University of Alabama at Birmingham Institutional Review Board (IRB). The approved IRB protocol number for the present study is IRB-300005613. Due to the COVID-19 pandemic, all interviews were conducted virtually; therefore, the study team received IRB approval to waive documentation of informed consent. Written informed consent was deemed unsafe due to the COVID-19 pandemic. Interview participants were given an information sheet during recruitment to review and ask questions prior to scheduling the interview. During the interview, informed consent was obtained verbally and recorded as part of the transcript. Similarly, patients participating in surveys were given an information sheet during recruitment to review and ask questions. In order to protect the identity of participating patients, we requested a Waiver of Informed Consent Documentation, which was approved by the UAB IRB. The information sheet was reviewed again and verbal consent was obtained before the patient was provided access to the survey.
