: This study has been approved by the Norwegian Centre for Research Data, ref. nr: 478838 and the Regional Committee for Medical and Health Research Ethics, ref. nr: 166280. The head of the different hospital departments gave permission to conduct participant observation and interviews. All methods were carried out in accordance with relevant guidelines and regulations. The Protocol for this qualitative study was approved by the Norwegian Centre for Research Data and the Regional Committee for Medical Health Research Ethics. All the staff present during observation were informed that they could decline from being observed at any time according to the ethics approval. All participants gave written informed consent to be shadowed after receiving written and oral information. All interviewed informants gave written consent to being interviewed and were informed that they at any point were free to redraw their participation.
