: The study protocol was approved by the Research Ethics Committee of Finnish Institute for Health and Welfare (decision number THL/442/6.02.01/2018 §787). Permission to use the register data was obtained from the Finnish Institute for Health and Welfare and Statistics Finland.This is a register-based study concerning pseudonymised secondary data of the total Finnish population for years 2010 – 2017, and according to Finnish data protection legislation, the participants are not to be contacted, but permission to use pseudonymised data can be obtained from the competent register authorities, i.e., from Statistics Finland and National Institute for Health and Welfare. All analyses were carried out in accordance with Finnish data protection legislation and research ethics guidelines and regulations provided by the Finnish National Board on Research Integrity. Information concerning the study, its goals, data and methods is publicly available on the internet-page of the Institute.
