: This study was deemed exempt from continuing review by the institutional review board at Yale University under 45CFR46.104 (2)(iii), a determination that extends to the eight study sites. Informed consent procedures will be followed when recruiting each site to participate, and when inviting participants (guiding coalition members) to the survey and interviews.
