: The study was approved by the Centre for Excellence in Research (CER) of the Goa Institute of Management. All methods were carried out in accordance with relevant national ethical guidelines and regulations. Permission for the conduct of the study was initially obtained from the National Health Authority, India, the nodal institution for PMJAY implementation and relevant scheme authorities in the respective states. Verbal informed consent was taken from all study participants prior to the conduct of interviews. Verbal consent was approved by CER in accordance with Indian Council of Medical Research 2018 waiver conditions for programme evaluations, as the study was solicited by policy makers, and as long as respondents were not individually identifiable in published reports. All participants were explained the study objectives and informed that their participation was voluntary, that they were free to decline to answer any questions, or exit the interview at any time. Verbal consent was taken additionally for audio- recording of interviews, and where not obtained, only detailed notes were made.
