: The University of Leicester Research Ethics Committee granted approval for the secondary analysis of qualitative data and the stakeholder consultation (including the procedure for gaining informed consent); the stakeholder consultation also received HRA approval (Ref: IRAS 209085). Details of the informed consent procedure are included in the Methods section. For the stakeholder workshop in Phase 2a verbal consent was provided at the start of the workshop. For the individual stakeholder interviews in Phase 2b, potential stakeholders who replied to the invitation to participate were sent a consent form by email; immediately prior to the telephone interview, the researcher audio-recorded their verbal consent. For the consultation exercise in Phase 2b (conducted at the Diabetes Update meeting) delegates who participated provided signed informed consent.
