: Ethics approval was not required for this study since the data is secondary and is available in the public domain. DHS data collection procedures ensured that all participants remained anonymous and gave informed consent to participate in DHS surveys. These procedures have been approved by the International Coach Federation Institutional Review Board (IRB) which ensures that the survey complies with the U.S. Department of Health and Human Services regulations for the protection of human subjects (45 CFR 46), as well as a country-specific IRB which ensures that the survey complies with laws and norms of the nation.
