: This work was designated non-research, and requirement for informed consent and IRB approval waived, under the terms of a signed attestation of non-research from the Veterans Health Administration Office of Primary Care (OPC). The OPC documentation ensures that work not carried out under a human subjects protocol is part of institutionally-sanctioned quality improvement activities.
