: This study, including the informed consent process, was approved by the Nepal Health Research Council (NHRC) (Reg no 36/2014), the Health Sciences’ Human Research Ethics Committee at the University of Cape Town, South Africa, as well as by the World Health Organization Ethics Review Committee, Switzerland (reference number: RPC 497). Informed consent was obtained from each participant after research staff read the informed consent to each study participant. Participants who were able to read and write provided their names and signature as written informed consent. Participants who were not able to read and write provided verbal informed consent in line with standard research practices in Nepal. We confirm all methods were performed in accordance with the relevant guidelines and regulations.
