: Ethical clearance was obtained from the Ethical Review Committee of the University of Gondar College of Medicine and Health Sciences, Institute of Public Health (Ref. No: IPH/1461/2013). Then, a permission letter was obtained from the respective public hospitals before the actual data collection. Written informed consent was obtained from each study participant. The autonomy and confidentiality of participants were maintained by using code numbers instead of personal identifiers. Finally, the study was performed in accordance with the Helsinki Declaration.
