: The study is based on a RCT that followed the declaration of Helsinki [CitationRef removed] and was approved by the Regional Committee for Medical and Health Research Ethics, South-Eastern Norway (2014/1719). The study protocol was registered at ClinicalTrials.gov (NCT02359149) 09/02/2015. The authors confirm that all methods were performed in accordance with the relevant guidelines and regulations.
