: All methods were carried out in accordance with the relevant guidelines and regulations in the Declaration of Helsinki on ethical principles for medical research involving human subjects. The DIPLOMA programme of research of which this study is a part of was reviewed and approved by the North West Greater Manchester East NHS Research Ethics Committee (Reference: 17/NW/0426, 1st August 2017). Participation was anonymous, and informed consent was given by return of the questionnaire. Contact details were requested to facilitate distribution of a £25 voucher. Survey data were anonymised on receipt.
