: This was a qualitative study with health care teams in primary health care organisations from rural and urban areas of Slovenia, whose willingness to participate in the study were confirmed by signing an informed consent form. This study was part of a larger project entitled “Scale-up an integrated care package for diabetes and hypertension for vulnerable people in Cambodia, Slovenia and Belgium” (SCUBY). The study within the overarching SCUBY project was approved by the National Ethics Committee of Slovenia (Ref: 0120–219/2019/4). All methods were carried out in accordance with relevant guidelines and regulations.
