: All methods in this study were carried out in accordance with the World Medical Association Declaration of Helsinki because we used data aggregated within the existing routine health information system, for which no informed consent was required. Because the data were aggregated, the dataset we accessed did not have any personal identifiers and therefore was anonymized. The University of North Carolina at Chapel Hill’s Office of Human Research Ethics (OHRE) determined that this study did not constitute human subjects research as defined under federal regulations 45 CFR 46.102 (e or l) and 21 CFR 56.102(c)(e)(l).
