: Analyses on the basis of the registry CEDATA-GPGE and the supplementary survey were approved by the Ethics Committee of the Justus-Liebig University Giessen (ethic approval protocol number 07/11) and by all ethics committees of centers involved (see Additional file ). Participating centers from Austria have an additional local ethics vote. In order for patients to be included in the registry, their parents must give their written informed consent.
