: Ethical clearance was obtained from the Institutional Review Board (IRB) of the Institute of Public Health, College of Medicine and Health Sciences, University of Gondar with Ref. No: IPH/1408/2013. All required permission letter was obtained from the Bench Sheko zone health department, woreda health offices, and respective health facilities. Written informed consent was obtained from each study participant. All methods were performed following the relevant guidelines and regulations, and the consent procedures were approved by the ethics committee. The anonymity of the participant was kept by informing them not to write their name and individual information was not disclosed to other persons or parties. Participants were informed regarding their full right to participate or refuse participation in the study. The study was done according to the Helsinki Declaration.
