: The study protocol was approved by the Research Ethics Committee of Finnish Institute for Health and Welfare (decision number THL/442/6.02.01/2018 §787). This register-based study used pseudonymised administrative data. Thus, according to national data protection legislations – Danish Data Protection Act (Act No. 502 of 23 May 2018), Finnish Data Protection Act (1050/2018), and Swedish Act containing supplementary provisions to the EU’s GDPR (SFS 2018:218) – the participants were not to be contacted and individual consent to participate was deemed unnecessary. Analyses were performed in accordance with the data protection legislation, research ethics guidelines and regulations of the three Nordic countries.
