: Ethical approval was granted by the National Institute for Medical Research (NIMR) of Tanzania with Ref. No. NIMR/HQ/R.8a/Vol.IX/1986 and Dalhousie University Institutional Review Board. Permission to conduct research in these facilities was obtained from the Tanzania Commission for Science and Technology (COSTECH) with Ref. No. CST/ AD.69/227/2015 and the President’s Office for Regional Administration, and Local Government. Patient’s consent was not obtained as this study was not designed to collect individual patient’s records and no author had direct interaction with patients at any point in time. The need for consent to participate was deemed unnecessary according to the National Institute for Medical Research regulations. During clinical audits, patients’ names were anonymized. All methods were carried out in accordance with relevant national guidelines and regulations.
