: The followed procedures were in accordance with the ethics standards of the Medical Ethics Committee of the VU Medical Centre and Academic Medical Centre. All participants took part voluntarily and signed informed consent. Confidentiality was maintained using restricted, secure access to the data and destruction of audio tapes following transcription. Ethics approval by a review board was not required by the Dutch Medical Research Involving Human Subjects Act. However, this was done voluntarily by living labs B (Institutional Review Board at the division of Pharmacoepidemiology and Clinical Pharmacology of Utrecht University with file number UPF 2009) and D (review board UPPER with file number UPF2106).
