Full ethical approval was obtained from Leicester Research Ethics Committee. Participants provided written informed consent, except where they lacked the capacity, in which case parents or guardians provided written informed consent and the young person provided written assent. Young people younger than 18 years were allowed to consent to participation in the study themselves if they were deemed by the study research nurses as having the capacity to consent. Research nurses were trained in being able to assess capacity to consent.
