: The RCT obtained ethical approval from the Regional Ethical Review Board in Gothenburg, Sweden (DNr 497 − 17, T023-18 and T526‐18) also covering the present sub-study. All participants have received oral and written information about the study and their rights as study participants and provided informed consent prior to engaging in any study activities. They were informed that their decision on participation would not impact on their usual care or their possibility to continue in the RCT.
