: For this study we used aggregated hospitalisation data supplied by the Australian Institute of Health and Welfare (AIHW). AIHW is an independent statutory Australian Government agency subject to the Australian Federal Privacy Act (1988) which regulates how public agencies can collect, hold, use and disclose personal information. The AIHW operates an Ethics Committee according to the Australian Institute of Health and Welfare (Ethics Committee) Regulations 2018 made under the Australian Institute of Health and Welfare Act 1987 (National legislation). Additional ethical approval for our study and informed consent were deemed unnecessary according to the above Australian national regulations. All the methods included in this study are in accordance with the declaration of Helsinki.
