: All methods were performed in accordance with the relevant guidelines and regulations, such as the Declaration of Helsinki, the guidelines by the Norwegian National Research Ethics Committee for medical and health research (NEM), and the Vancouver Protocol. Participation in this study is based on voluntary, informed consent, and the participants are anonymous. Informed consent to participate was taken from all participants and from parents/legal guardians in case of minors. One participant was below 19 years of age, but older than 16 years old. The scope of the study was deemed to be outside the scope of the Norwegian Health Research Act. Consequently, the Regional Committees for Medical and Health Research Ethics sør-øst C study granted an exemption from requiring ethics approval (ref. no. 270172). The Privacy Ombudsman considered the data to be anonymous (ref. no 599674) and the data were stored according to the current regulations and local procedures at OsloMet University.
: For the images used in this manuscript, an informed consent is obtained from all individuals. Both individuals that are identifiable have given their informed consent to publish the images in an online open-access publication. None of the individuals are actual patients and the situation is staged for the purpose of giving an illustrative example. Consent to publish the image is also obtained from the photographer.
