: This study was performed in accordance with the principles of the Declaration of Helsinki. The Southern Adelaide Clinical Human Research Ethics Committee approved the study (ID 313.17). For minors (aged ≤ 16 years) written informed consent for participation was obtained from a parent/legal guardian. For young people 16–17 years written informed consent was obtained from the young person and verbally from their parent/legal guardian. Adults (≥ 18 years) provided written informed consent for their participation. The trial was prospectively registered ACTRN12617001589370 (28/11/2017).
