: This study entirely relies on de-identified administrative data that is publicly available for research purposes. No human data have been used. Permission to use German inpatient data (DRG statistics, to ) has been obtained from the Research Data Centre of the Federal Statistical Office (). Permission to use U.S. inpatient data (NIS, ) and emergency department data (NEDS, ) has been obtained from the HCUP Central Distributor (). Institutional review board approval and consent to participate was not required in accordance with the U.S. HHS regulations for the protection of human subjects in research at 45 CFR § 46, and in accordance with the German guideline for conducting administrative data analyses “Good Practice in Secondary Data Analysis (GPS)” (Gesundheitswesen. 2015;77(2):120–126).
