: As stated in the manuscript, in Denmark there is no requirement for ethical approval if no data refer to identifiable persons or institutions. This study was reported to the record of processing activities for research projects in the Central Denmark Region (record nr. 160406). It followed the principles of the Declaration of Helsinki and complied with all requirements to ensure the safety of the human volunteers. The heads of the departments of primary care and the individual participants provided informed written consent. The observations contain no information that could identify the participants or the outpatient clinics included in the study.
