: The study was approved by the Norwegian Agency for Shared Services in Education and Research (Ref. 837886) and the main project was approved by <i>Regional Committee for Medical Research</i> (REK-VEST 2011/1609). Informed written consent was obtained from all participants. The written information specified that participants were informed that their contribution was voluntary and that they could withdraw from the study at any point without consequences. To ensure the participants’ anonymity, we used numbers instead of names in the transcribed material. All methods were carried out in accordance with relevant guidelines and regulations.
