: Study procedures were approved by the Indiana University Institutional Review Board and the VAMC Research and Development Committees at each study site. Informed consent for observations and the recording of interviews was verbally obtained from participants prior to study activities. As approved by the Indiana University Institutional Review Board, participants were provided with a Study Information Sheet. Study personnel reviewed the information sheet with the participant prior to starting the scheduled study session. A waiver of consent was approved to access medical records involved in the study; this research presented no more than minimal risk.
