: All methods were carried out in accordance with relevant guidelines and regulations. The method consisted of a survey for which written informed consent was obtained from all participants (i.e. physicians). Prior to participation, respondents were informed via the invitation that the results of the survey would be published. Participants were not subjected to any procedures and were not required to follow rules of behaviour, meaning the Medical Research Involving Human Subjects Act (WMO) does not apply to this study and approval of the Medical Research Ethics Committees United (MEC-U) was not required. It is important to note that, as per the Dutch national legislation, especially the Medical Research Involving Human Subjects Act (WMO), ethical approval from an institutional review board or committee is not mandatory if the participants were not subjected to any procedures and were not required to follow rules of behaviour.
