: Ethical approval was sought through the participating study site hospital and university, but it was judged that this project met the National Health and Medical Research Council requirement for being a quality assurance/audit project (in accordance with the Australian Government National Statement on Ethical Conduct in Human Research 2007 []). As such it was approved by La Trobe University Ethics Committee (approved 4th April 2016) and the study sites The Royal Women’s Research Committee and Human Research Ethics Committee (approved 3rd May 2016). All potential respondents of the participating maternity services were informed about the goal, procedures, risks, benefits, anonymity of data and alternatives for participating in the study via written study information included in the email invitation, via discussion in clinical handovers, in-service education, and team meetings, and via paper flyers so that they could make an informed decision about participation. Participation in the survey was considered as having obtained informed consent from participants.
