: The study was approved by the Norwegian Center for Research Data (ref. no. 141435). All participants received written and verbal information about the study. All participants were given the right to withdraw from the study at any point during or after the interview. All signed a written consent to participate prior to the interview starting. All responses were given pseudonyms during the transcript process and the information required to map deidentified data with personal information was stored at a separate, encrypted location. To protect participant anonymity, their characteristics are reported in intervals and hospital identities are not given.
