: This study is classified as operational research. Permission was sought from The Ghana Field Epidemiology and Laboratory Training Programme (GFELTP), a collaboration between the Ghana Health Service and the University of Ghana’s School of Public Health. Permissions were additionally sought to evaluate the maternal death surveillance system through the Bono Regional and Sunyani Municipal Health directorates. All participants provided written informed consent before they participated in the study. No participant was below the age of 16 years. Participation in the study was voluntary, and participants were assured of confidentiality and anonymity. Written consent was obtained without any form of coercion or undue influence.
