: This mixed-methods study (i.e., surveys and interviews) was approved by the Vanderbilt Institutional Review Board (IRB) (IRB# 211641) and by the local IRBs for each research site. Informed consent was obtained from all survey participants using an IRB-approved informed consent form in REDCap. GUIDED was delivered in the real-world setting as part of usual care for all patients with HF admitted to the emergency department. This study adheres to the EQUATOR network reporting guidelines and research checklist of “Mixed Methods Reporting in Rehabilitation & Health Sciences.”
