Ethics approval and consent to participate
: Our study sample consisted of healthcare professionals questioned about their profession, and therefore ethical approval was not required as this is not subject to the Dutch law of medical scientific research (36). All participants volunteered. Informed consent was assumed when participants started the online questionnaire. All interviewees returned a written informed consent form.
