: This study was approved by the Tokyo Metropolitan University Research Safety and Ethics Committee in 2017 (approval number 17034, approved on July 25, 2017). The need for ethics approval and written informed consent was waived by the Tokyo Metropolitan University Research Safety and Ethics Committee since this study was non-invasive, non-interventional research and did not use samples obtained from the human body. In the personalized letters, the participants were informed that they were under no obligation to complete the questionnaire. Returning the questionnaire was taken as consent to participate.
