: Ethical approval for the study was obtained from the national ethical review board of Nepal Health Research Council (Reg. no. 371/2022). A letter of support was issued from the Health Insurance Board and was circulated to all the health facilities before initiating the data collection process. The description of the study was provided orally to all the health facilities and coordination to the participants was done accordingly. Along with this, a written informed consent (thumbprint in presence of witness for those who could not provide written consent) was obtained from the participants prior to the data collection. The consent included the details of the study purpose, expected roles of the participants, clear communication about the benefits of the study and autonomy of participation in the study. The study adheres to the ethical principles outlined in the Declaration of Helsinki.
