: The study has ethical approvals from the Norwegian Centre for Research Data (NSD 383963), the Nepal Health Research council (NHRC-ERB Protocol No. 455/19) and the Institutional Review Committee of Kathmandu University School of Medical Sciences/Dhulikhel Hospital (IRC-KUSMS 281/19). Participants received verbal and written information and written informed consent was obtained from both the adult participants and the parent (s)/guardian (s) of all under the age of 16. Participation was voluntary, and participants were free to withdraw from the study at any time for any reason without having to declare why. Data were anonymised by omitting the use of any identification key connecting the survey response to the person. The survey did not contain any identifiable data that could connect the participant with the area or health station/hospital. Handling of personal data was in accordance with the EU General Data Protection Regulation GDPR and NSD regulations on privacy policy. The study was performed in accordance with relevant guidelines and regulations in the Helsinki declaration.
