Ethics approval was obtained from the South Metropolitan Health Service (RGS0000003058) and Edith Cowan University Human Research Ethics Committees (2019 − 00989). Informed consent was obtained from all participants in the qualitative components of this study. For the quantitative data, which involved retrospective auditing of de-identified medical records, the South Metropolitan Health Service Human Research Ethics Committee granted a waiver of consent.
